Initial Comments:
Based upon the findings of an off site follow up survey conducted on 4/23/2021, Apex Home Healthcare, corrected the deficiencies and was found to be in compliance with the requirements of 28 Pa. Code, Part IV, Health facilities, Subpart G. Chapter 601.
